The previous responders are correct. A schedule II drug can only be legally obtained by a written prescription. A doctor cannot just call it into the pharmacy or OK a refil. Electronic transmission has only partly been approved by some state pharmacy boards.

You probably could find a source, but the possession would be illegal, a federal crime, and more expensive.

There is Vicodin and there is Hydrocodone. It is the exact same thing. One is more expensive because it is being sold under the name brand, Vicodin. No different that if you buy Tylenol, or if you buy a bottle of the cheap stuff that says Acetaminophen. It is all the same thing. You do not have to have external bleeding. External bleeding is when blood has exited the body as some point. The brain is very fragile and can bleed or bruise very easily. That is why your skull is so hard. You can damage your brain without injuring your skull-closed head injury. It happens when the force is so great that your brain is bounced around inside your skull. The bleeding or swelling can damage thoise areas in the brain that control other parts of the body- lungs, heart, kidneys etc.

It depends on your personality. When I did my Medicine rotation in med school, I seriously questioned whether I made the right choice in going to med school.

I would rather poke sharp things into my eyes than sit through Medical rounds. Seriously.

I found surgery to be more to my liking, but when I discovered anesthesia, I found my home.

There are plenty of doctors that would be miserable doing what I do, but I love it.

You don't really choose a specialty - it chooses you. If you like Medicine, then it's a good medical specialty. We desperately need good internists. If it's not for you, then it's not a good specialty. Something else will be, though.

Yes, your symptoms are consistent with a slowing thyroid. Some of these symptoms could also be attributed to problems other than hypothyroidism. For thyroid performance a simple blood test can be performed. It's not unusual for younger people to develop thyroid problems. If that is the problem you will need your doctor (GP) to recommend you to an endocrine specialist.

If hypothyroidism presents, treatment usually consists of a thyroid supplement that you will take once a day for the rest of your life. It's not a big deal, but the medication can be expensive. It might be a good idea to make sure you have health insurance and a good drug plan in place before you see the doctor. It will be more expensive after you have been diagnosed, because you will have to disclose the condition!

One symptom that you did not describe would be mood swings. For instance people with slowing thyroids often have a problem in their relationships which, untreated, can lead to divorce.

The dry skin is more consistent with simple advancing age. It could be treated with lotions or if extreme cortisone cream. Since you are only 18, you'll need to ask your doctor about that.

Topical cocaine is still used regularly to constrict the sensitive blood vessels inside noses during certain nasal operations to decrease bleeding. There is also a topical mixture called TEC for short which is tetracaine, epinepherine, and cocaine that is used on people's skin in some emergency rooms to numb up an area for stitches. This is used often in children so you don't have to hurt them by giving them a shot of lidocaine to numb up their skin.

As a veterinary student I like animals. A lot. This is such a complicated issue that I was asked about it during my interviews! Unfortunately, as said above we do not have a choice if we want to "cure" people and while there are DEFINITE differences, most mammals work the same. (and scientists know what those differences are)
Something else to note, Rechigo, is that animal testing has to be done to help cure animals too! In my class even strict vegetarians with strong animal rights ties reluctantly agree about this point. It's not fun but in our training we even have to dissect dogs, cats, horses, and cows etc to know how they work when we help your animals. What we all can do, however, is make sure researchers follow the laws and guidelines that govern animal testing and try to use different methods whenever possible.

I think it is great that you are asking questions about issues that trouble you. When I was in junior high I cried when we dissected a baby pig - so I can see where you are coming from! In the future though maybe try and find answers from other sources like .org or .edu websites.

No, not unless you have some symptoms, a potential diagnosis, or a family history of something significant that would require an MRI. Find a good doctor to take care of you and if he/she thinks you need an MRI, then get one.

What does it cost? If you search the web, you'll find the price advertised. There are full body CT scans (note: not MRI) that are around $500. CT scans expose you to some low levels of radiation, while MRI machines use magnetic fields to make their images. MRI tends to be more expensive than CT.

An MRI (or a CT) can detect a lot of things. That's really the problem. If you do a full body scan (without any symptoms), then you may be tempted to think that any finding is bound to be serious. Sometimes a slight blemish on a scan is just a poor image and less frequently, it is a lesion or a tumor.

Iron sucrose side effects? Is it dangerous?

My mum has chronic kidney disease for the past 14 years. Her health has been well, her blood urea and creatinine have been under control. However, her haemoglobin level was low and ferritin test showed that she had iron deficiency. So, she was asked to take the iron sucrose medication. While she was taking the iron sucrose medication, she started to feel tightness in her muscle, she started getting back pain and stomach ache. She also felt and weak and her blood pressure was very low. The doctor immediately stopped the iron sucrose medication.After a few hours, my mum got better, her blood pressure was normal and her muscle tightness improved. I just want to know whether her health will be damaged due to the side effects? Especially her kidneys, will it get damaged due to the side effects?

I am an MD. The symptoms that you describe are entirely temporary. There will be no permanent damage to any part of her body, I assure you. The Iron sucrose is a great idea and you should ask your doctor to try it again, this time with a little Valium or Ativan on board before the infusion begins. Chronic kidney patients can't absorb Iron easily from their GI tracts, making IV administration the only viable option. Correcting her anemia will have wonderful benefits to her health, including keeping her heart stronger for longer, as well as giving her more energy and a better outlook on life. She will be far more damaged from the lack of Iron than anything the Iron sucrose might cause. And if she is on dialysis, her kidneys are gone anyway. If she is not on dialysis yet, then correcting her anemia will enhance and prolong the life of her kidneys. I hope this helps.

I am taking my recerts this year and was wondering if any one has any thoughts on how difficult/easy the exams are, particularly for sub specialists.

As you probably know, the ABIM has a multipronged path to recertification, and I assume you're talking about the proctored exam. Most people (even subspecialists) pass it. Of course they only tell you what the pass rate is for any given exam AFTER you take it (you'll get that info with your exam score), but in general %26gt;85% sail on through. It may or may not give you confidence, but remember that the quoted pass rates are brought down by repeat takers with at least one failed exam under their belts. They tend to do worse, so the effective pass rate for all the others is extremely high - I'm guessing in the neighborhood of 95%.

For the treatment of bulimia nervosa, the recommended dose is 60 mg/day, taken in the morning. Prozac doses above 60 mg/day have not been systematically studied. So i would recommend that you back it back down to the prescribed dose because even though you may feel great and have no side effects you may be destroying your liver and when the liver goes we go. Worry about getting into medical school first. While in med school, you'll rotate through all the specialties and you may realize surgery is not for you.

As far as college goes, there is one thing I would recommend. Don't major in Biology just because you're going to med school. Bio majors are a dime a dozen, and many of them have mediocre grades because they majored in something that did not interest them.

The year I got accepted into med school, the majors with the highest acceptance rate were Philosophy and History. Each around 70% of their applicants accepted, however, there were only about 400 applicants.

Consider this, there were 15,000 Biology majors who applied to med school and their acceptance rate was around 30%.

I'm guessing there were a lot of bio majors with 2.5 GPAs. while all those Philosphy and History majors had 3.5s or higher.

Med school admission committees like to see a high GPA, it means you work hard at what you like.

Unfortunately there is no definate diagnosis able to be made for Alzheimers while alive - it can only be confirmed through an autopsy.
Your friend should see a Neurologist who will do some cognitive tests and can give you an idea of how far advanced his dementia is.
It sounds to me from my personal experience that he's still in the early stages. It's common that they can become aggressive and rather mean and spiteful to grandchildren they do truly love. Noise is a problem as is light. A common problem is called "sundowning" and it happens, as in it's name, around the time the sun starts to go down. I recommend painting in bright, light colours any rooms that have minimal light and have lights on by dinner time (earlier in the winter).
Speak to this person and discuss their problem with them. Things have to be decided in advance. There will come a time no matter how much you know (and I was trained medically to deal with this) that you will no longer be able to cope 24/7 and the person will have to be transferred to a nursing home. Let them help decide which one they would prefer and maybe visit some.
Our police department has a special program where Alzheimers patients register and wear a special bracelet (like a medical alert one). The places they are most likely to wander to are known to police and they know how to approach this person if they go missing, without scaring them. You may want to look into that.
Once dementia moves along the person will start to wander and cannot be left alone. If they are, remove fuses from the stove, hide the kettle, anything that could start a fire while you are out. At night, in case you don't hear them...place a lock either at the very top of your doors or at the very bottom.they won't look there. After unlocking the door they still won't be able to get out so may just go back to bed.
They will also start to forget people and current events in their lives although they will remember things from the past like they just happened. It's very hard on loved ones to see this...that's why it's called "The long goodbye".
Near the end stages their memory will become worse and they may experience either/or expressive or receptive ephashia. That means they can either no longer talk or no longer understand what is being said to them. They may also start to have trouble swallowing food and have to go on a diet of soft blended foods.
If they are not exercised by the staff (and most nursing homes are understaffed and don't do this) they may develop contractures...their legs crossed and "stuck" in that position (or hands, arms positioned in an odd way).
At midpoint of the disease they will lose control of their bladder and then bowels making adult diapers necessary.

It sounds like your friend is in the early stages...please get him to a doctor asap for a diagnosis. There are programs that family and friends can attend put on by the Alzheimer Society which will tell you exactly what will happen as the disease progresses. Your best friend is knowledge.

Sometimes we do a scan with and without contrast because the brain can be visualized differently The Radiologist will report your scan the same day ( or the next day ) but then the report has to be typed and sent. I am a rising sophomore at one of the most competitive schools in the nation, and I hope to go to medical school in the future. I sort of lost focus in the second semester of my freshman year. Next semester, I will resist all sorts of temptation and just focus on my work. I will put social life aside. I plan on doing many things: studying my butt off, getting involved in an undergrad research project, being a TA for general chem, becoming a member of this student organization called Community Health Advocacy Program, and working out, which is like my only way of dealing with stress. These are things I want to and must do. However, I am not so sure if i am capable of balancing out my time for all of those activities. The life of a pre-med is indeed tough. Please give me some tips on how to distribute/manage my time.

Indeed it is not easy but it's not an impossible to do. I've been there. Take time off when you can and trust me you'll have the time and enjoy yourself. Study, research repeat may get you into to med school but results in a crappy doctor. Try to have other experiences and do what you enjoy and have a passion for...make yourself unique and different. Don't think XYZ will look good I should do it, believe me there is not a set of magic recipe to get into med school.

I've interviewed dozens of applicants to med school and I've recommend many types of people from the Art History major who spent two years with the Peace Core to the Engineer who's been in the industry for a few years to the kid straight out of undergrad.

All the ones I've recommend have had genuine enthusiasm in the stuff that they did. Like if you tell me you did research or volunteer or whatever, ok that's fine...I follow it up by why did you do it. That answer for me usually determines the positive or negative recommendation, if I get the sense from a BS answer that it was just an application builder, boom instant negative. But if someone comes back with genuine excitement or passion about the experiences that they had and how it taught them or it affected them in someway in any way, instant positive. People who experience life are the ones I recommend and not simply someone who does things to be the "ideal pre-med"

As for your specific schedule, research, TA, joining the Advocacy program are all fine things to do but ask yourself why do I want to do this. If you view them as learning experiences to build you as a person then do it, if you're doing it to impress some future admission committee, don't do it, do something else.

If you research the drug you can find out its bioavailability, LD50(amount to take to kill you), how its metabolised etc.There are 2 phases in drug metabolism and it depends on the drug what route it takes so the by products would be different. Drugs after metabolism wouldnt noramally be harmful (thats why you take them in the first place) because its normally deactivation of the drugs active site and so it wouldnt be harmful but adverse drug reactions can occur due to chirality (kind of like a spatial change in the molecule). Why become a surgeon? There's less BS than in internal medicine. You find a problem and you fix it. Plus, you get to cut and sew, and those things are fun.

Why NOT become one? Still too much BS. There's a lot of post-operative messing about, pre-operative messing about, finding ways to transfer problem patients off of your service, chasing down lab work and xrays. and the training takes forever.

I still get to play in the OR every day, and I get to stick large needles in people on a regular basis, but don't have to deal with nearly as much garbage. (If lab work isn't available, I can throw it back into the surgeon's lap!) And, when the cases are done for the day, I get to go home and see my family instead of making rounds.

If you're not past your third year of medical school, it's not a decision that you need to make any time soon. You have to spend three months in surgical rotations before you know if you love it or hate it (there really isn't much middle ground).

If someone were to down these medications with a full bottle of Smirnoff... medically, what should happen?

180 mg of dextroamphetamine (adderall xr), 10 pamprins, 10 tylenol, 4 excedrins and 8mg of risperidone (riperdol).

the pamprins, tylenol, excedrins, and risp would probably make the person very sick. They wouldn't kill someone, but would put them out of commission for a few days and could results in some permanent sickness like liver damage.

The alcohol would not react at all with the other medications, it would just make the person drunk if it's a big bottle - possibly making the person even more sick and complicating the nausea.

The adderall... I'm not sure how much narcotic is found in adderall, but 180 mg sounds like quite a bit and could be dangerous. However, it depends on the narcotic and all of that of course.

Not enough to kill a healthy person if they took it all at once, but possible could kill someone due to the complications of having multiple sicknesses at one time!

It would hurt like hell, for one - but if someone wanted to commit suicide it would be slow and risky way to get the job done.

Hopefully an MD can come on here and go into more detail.

If someone dies during surgery, should the family still have to pay everything?

If someone dies during surgery, and the surgery isn't even completed, should the doctor still charge the deceased loved ones the entire cost of the procedure? Or due to ethics, should they write off at the very least, the co-payment (the portion the insurance doesn't cover)

Furthermore, would the insurance even pay for a surgery if someone passes away during it? I would think they wouldn't. Well, then the life insurance policy (if they have one) would pay for it, I guess.

Secondly, what DO most doctors do? What SHOULD they do vs. What DO they do? Do they do what's ethical and write off the costs?

Fortunately, death during surgery is very rare.

In those cases where it does occur, it is usually not the result of a "botched" operation (think, gunshot wound, emergency bypass surgery, etc.) Those cases require HUGE amounts of resources and are very expensive. Are you suggesting that when it matters the most, doctors should work for free? Those cases are physically and emotionally demanding on the people who care for the patients.

Insurances do cover the costs associated with surgery, and it is not related to the outcome.

Risk management concerns come up if a patient dies because of a medical error. It is not in the errant doctor's best interest to bill for a procedure that he/she screwed up, and I doubt many would.

If doctors were paid based on outcome, how would the difficult cases ever get done? Who would take the risk? It's bad enough that lawyers are ready to sue at the drop of a hat, even if the care delivered was perfect.

Sometimes bad things happen. There are NO guarantees in medicine.

What would you say if the reverse was suggested? If the patients don't follow the doctor's instructions and something goes wrong, they pay double. (hello, smokers!) Just a thought.

I have some friends that are pretty accurate at guessing when the rain is coming by the feel of the pain in their arthritis.

If psychologists were ever able to prescribe medicine, would they see a profit from it?

I'm kind curious for the reasons they are pushing for psychologists to be able to prescribe medicine. Would they make a dime off of it?
Answer:
Psychiatrists can prescribe medication but do not make any money off it. You pay the pharmacy for the medicine, not the doctor. Psychiatrists sometimes make money from pharmaceutical companies by carrying (and distributing) those free samples, but that's it.

I would imagine psychologists, if they could prescribe medications, would be no different.

If oxygen isn't flammable, why do people care if people smoke around a person with respiratory oxygen therapy

The oxygen itself will not burn, but the oxygen mask or cannula, clothing, skin, hair, and anything else that can act as a fuel WILL.

We have to be careful in the OR because it is a high oxygen environment, and things like surgical drapes and endotracheal tubes will combust if a laser or cautery ignites them. Airway fires can be awful, even deadly.

There is also the issue of not smoking around people with lung disease, so as to not further cripple their lungs, but that has nothing to do with flammability.

If my paternal grandmother was a twin, what are the chances that I will bear twins?

If you can go back and see if there are more twins in the family. Then the chances are better for you to have twins...
But if grandma was the only set of twins then your chances arent as great...

A very large majority of capsules are made form some form of gelatin. Gelatin is a protein and is first broken down in the gastrointestinal system by enzymes specific for cleaving peptide bonds. (Proteins are compiled of peptide chains.) These enzymes include pepsinogen, pepsin, trypsinogen, and others. The resulting peptide fragments are easily absorbed in the small intestine given the large surface area, and then are taken up into circulation.

If I skip taking my Wellbutrin SR for the day will it be ok to drink alcohol ?

Wellbutrin has a relatively long half-life of 24 hours. This means that even if you skipped your dose you would still have the drug in your system and probably will have it in your system for at least two weeks. Not to mention, Wellbutrin doesn't work as well if you skip doses, this is a medication that has to build up to a therapeutic dose in your system and can be affected by missing a dose or being late to take it. While drinking alcohol would increase your risk for seizures and other ill side effects, you can use your best judgement. As a healthcare professional, my suggestion is to steer clear of the alcohol to avoid any possible problems. You can still have a great time even if you don't have a drink. It's ultimately up to you, you know your body. If you start to feel bad, stop drinking and tell those closest to you to keep an eye on you. In case of a seizure, make sure that those around you know that all they need to do is make sure you don't hit your head. Anything else you may have heard about seizures is myth.
